Work Permits Procedure

Work permits are available for students ages 14-17 years old.

The Permit Application can be completed electronically or in person with a paper application. The Permit Application must be signed by the employer, a parent / guardian, physician if under age 16, the applicant in order to be processed.

Step 1: Secure employment
Step 2: Complete the Work Permit Application electronically or in person.

 

Electronically 

A valid email address is required from the applicant, employer, parent, & physician if under the age of 16.

Students who are 14 or 15 years old, click here.

Follow instructions to electronically complete & sign the Applicant portion of the form

  • Upload a valid proof of age document (only need one):
    • School ID
    • Copy of Birth Certificate
    • Driver’s License / Permit
    • US Passport
  • Need work employer email, parent email, & doctor office email if under 16
  • If you are not a student with Fairhaven Public Schools (FPS), a report card or letter of good standing from your current school will be requested via email
  • ReadySign will then email your Employer, the Parent / Guardian, & Physician if under 16, to complete & sign the appropriate sections
  • You will receive a copy of the completed application once all parties have signed off via email. PLEASE NOTE THIS IS NOT A WORK PERMIT
  • FPS will send a drafted work permit to you for signature via ReadySign. PLEASE NOTE THIS IS NOT YOUR WORK PERMIT
  • FPS will send the fully completed permit signed by an authorized agent to you via ReadySign. THIS IS YOUR PERMIT!
  • Print or Email a copy for your Employer

In Person

  • Complete all sections of the FPS Work Permit Application:
    • Applicant
    • Employer
    • Parent
    • Physician (if under 16)
  • Make an appointment with the FPS Administration Center by emailing [email protected]
    Hours: Monday through Friday, 8 AM - 3 PM, excluding holidays
  • What you will need to bring with you:
    • Completed application
    • Valid proof of age document as stated above
    • If you are not a student with FPS, a report card or letter of good standing from your current school
    • You & your parent/guardian must be present to obtain the permit


Work Permits will only be issued to students who will be working in the state of Massachusetts. If the employer is located in another state, please use that state's official website to view their labor standards/ work permit process.  The state of Massachusetts requires all minors between the ages of 14 through 17 to have a work permit for each employment. If you get a new / different job, a new work permit is required.
